Minimum Balance Requirements.
You must deposit $100.00 to open this account. A minimum balance fee of $4.00 will be imposed every statement cycle if the daily balance on any day of the statement cycle falls below $750.00. You may avoid the imposition of the minimum balance fee by receiving one ACH direct deposit each statement cycle. An ACH is an electronic deposit to your account by another party. Transfers and ATM deposits do not qualify.
Dormant/inactive Account Information.
A dormant account fee of $5.00 per statement cycle will be charged after 18 months of inactivity. Dormant account fee will be assessed if balance falls below $100.00.

Please refer to the separate Fee Schedule provided to you with this disclosure for information about fees and charges associated with this account. A Fee Schedule will be provided to you at the time you open an account, periodically when fees or charges change, and upon request.
A paper statement fee of $2.00 will be imposed each statement cycle. To avoid the imposition of this fee you may receive eStatements.
All charges are subject to sales tax.
